Introduction & Overview
The Mustafa K. Başkaya Laboratory at the University of Wisconsin–Madison is dedicated to advancing neurosurgical science through a dual focus on microsurgical education and innovative clinical and translational research. Our work integrates advanced anatomical studies, surgical training models, and outcome-based clinical research to improve patient care and to train the next generation of neurosurgeons worldwide.Our laboratory has three main pillars:Microsurgical Education & Training
- Development of high-fidelity training models for cerebrovascular bypass and skull base surgery.
- Development of “cocept of ultra-microneurosurgery”.
- Establishment of structured training curricula, including the Baskaya Microvascular Bypass Training Curriculum (BMBTC).
- Global expansion of microsurgical training opportunities through the Madison Microneurosurgery Initiative (MMI), which has reached 85 centers in 45 low- and middle-income countries.
- Since January 2006, we have hosted over 400 international medical student, neurosurgery residents and fellows, practicing neurosurgeons and anatomists from more than 48 countries, fostering worldwide collaboration in neurosurgical training.

With more than 320 peer-reviewed publications, including recent landmark work on global microsurgery training, surgeries of deep seated so-called inoperable gliomas, skull base tumor surgery, surgical management of aneurysms, arterio-venous malformations (AVMs), cavernous angiomas, and anatomical innovations, the Başkaya Laboratory is recognized internationally as a leader in neuroanatomical education, microsurgical excellence, and global neurosurgery capacity building.

Development of novel training models, curricula, and simulators for cerebrovascular bypass and skull base surgery; advancing global access to neurosurgical skills training, especially in low- and middle-income countries.

Detailed microanatomical studies and 3D cartography of safe surgical corridors, white matter tracts, and craniovertebral junction anatomy to guide safer neurosurgical approaches.
- Cerebrovascular Neurosurgery
Surgical treatment and outcome analysis of complex intracranial aneurysms, arteriovenous malformations, cavernous angiomas and rare vascular malformations; development of novel classification systems with direct clinical applications.

Clinical Neurosurgery Research
The Clinical Neurosurgery Research Program in the Başkaya Laboratory focuses on advancing the understanding and treatment of complex cerebrovascular, skull base, and other central nervous system lesions. Our work integrates anatomical insights, innovative surgical techniques, and clinical data analysis to improve patient outcomes and expand neurosurgical knowledge.Key areas of investigation include:
Cerebrovascular disease — participation in multicenter studies evaluating outcomes of surgical, endovascular, and radiosurgical treatment for intracranial aneurysms and arteriovenous malformations (AVMs).
Skull base and intracranial tumors — refinement of operative strategies for skull base tumors (meningiomas, chordomas and chondrosarcomas), deep seated complex gliomas (insular, thalamic and brain stem) and other challenging lesions through clinical experience and anatomical studies.
Novel anatomical and surgical classifications — contributions to the development of practical systems such as Sylvian fissure subtypes, parasagittal bridging vein patterns, and foramen magnum meningioma topographies that inform surgical planning.
Clinical outcomes research — identification of perioperative risk factors, predictors of complications, and long-term functional and neuropsychological outcomes following treatment of CNS lesions.
Global neurosurgery contributions — participation in collaborative efforts to evaluate and expand access to microsurgical training and education worldwide, with a focus on sustainable and affordable solutions.
Through these projects, the Başkaya Laboratory actively contributes to shaping evidence-based management strategies for some of the most complex conditions in neurosurgery.
Neuroanatomy Research
The Neuroanatomy Research Program in the Başkaya Laboratory is devoted to advancing the microsurgical understanding of the human brain and skull base. Using cadaveric dissections, high-resolution imaging, and 3D reconstructions, our studies refine surgical corridors and safe entry zones for complex neurosurgical procedures.Areas of focus include:
- White matter tractography and diffusion tensor imaging to define surgical safety zones.
- Microanatomical dissections of the thalamus, temporal lobe, craniovertebral junction, and orbitofrontal regions.
- Anatomical and radiological analysis of venous, arterial, and cranial nerve variations with direct implications for cerebrovascular and skull base surgery.
- Development of 3D surgical cartography to improve spatial orientation and surgical planning for deep-seated lesions.
- This research not only deepens the understanding of neurosurgical anatomy but also directly informs clinical approaches to aneurysms, AVMs, meningiomas, gliomas, and other challenging pathologies.
Neurosurgery Operative Skills Laboratory
The Lincoln Ramirez Neurosurgery Operative Skills Laboratory, directed by Dr. Mustafa K. Başkaya, is recognized internationally as a premier center for state-of-the-art neurosurgical training. The laboratory is uniquely equipped to provide students, residents, fellows, and international colleagues with the highest level of hands-on experience in microneurosurgery and skull base surgery, preserving the tradition of cadaver-based neurosurgical education pioneered by Professor M. Gazi Yasargil.Key distinctions include:
Global Impact – Since January 2006, the laboratory has welcomed over 400 international students, neurosurgery residents and fellows, practicing neurosurgeons and anatomists from more than 48 countries, establishing it as a hub for worldwide neurosurgical education, neuroanatomy and collaboration.
Comprehensive Training – Trainees master advanced techniques in cerebrovascular bypass, skull base approaches, complex tumor resections and ultra-microneuropsurgerywith direct application to the operating room.
Annual Cadaveric Courses – Each year, the laboratory hosts six advanced microsurgery courses for residents and one dedicated course for medical students, all provided free of charge. These courses bring together participants from around the world, offering unparalleled access to hands-on cadaveric training in a collaborative environment.
Cutting-edge Infrastructure – Multiple fully equipped dissection stations, each with operative microscopes, drills, and downdraft cadaver tables, enable simultaneous training. Integrated audiovisual systems allow live demonstrations and interactive teaching at the highest educational standard.
By combining world-class facilities, a strong tradition of anatomical dissection, and a global network of trainees and alumni, the Operative Skills Laboratory has become a model for neurosurgical education that inspires excellence and shapes the future of the field.
